Patient-Centered Primary Care Home Program

Patient-Centered Primary Care Home Program

The Patient-Centered Primary Care Home Program is part of Oregon's efforts to fulfill a vision for better health, better care and lower costs for all Oregonians.

Providers

Patient-Centered Primary Care Homes are clinics that have been recognized for their commitment to quality, coordinated care. At its heart, this model of care fosters strong relationships with patients and their families. Clinics improve care by catching problems earlier, focusing on prevention, wellness and management of chronic conditions.

List of recognized clinics

Become Recognized: As a recognized primary care home, your clinic will be part of Oregon's new direction to change the way we deliver care to improve health and lower costs.

Standards for Recognition: Recognized primary care homes are accessible, accountable and offer comprehensive, continuous and coordinated care to patients and their families.

Payment Incentives: Recognized primary care homes can now apply to receive additional Medicaid funding to support the patient-centered care they offer Medicaid patients with chronic conditions.

Families & Patients

In a Patient-Centered Primary Care Home, you're the most important part of your care.

Your primary care home will:

  • Better coordinate your care to help get you the services you need, when you need them
  • Listen to your concerns and answer your questions
  • Offer after-hours help and alternatives to the emergency room
  • Help you play an active role in your health
